REMOTE DERMATOLOGY SERVICE – FOR EXISTING PATIENTS

I am an existing patient of Rao Dermatology, having visited the Alberta DermaSurgery Centre in the past 12 months.  If not, I will NOT be permitted to use this service.

As an existing patient of Rao Dermatology, I would like to do the following:

• Schedule a follow-up appointment
• Ask specific questions about my existing health condition and/or management
• Request to fill out/complete paperwork
• Request a prescription renewal
